Please note that the downloadable registration form includes mathematical features which will calculate the total bill. This feature might not function on older versions of Adobe PDF (version 7). In this case either download the new Adobe version (see below) or use the form without the special features and calculate the total amount yourself. Then please print the form and fax it to Laura Alice for credit card processing.
